Employee is currently not enrolled in his own - rabiasr1996 - 10-07-2025 State-sponsored medical insurance plan but he is currently enrolled in his own State-sponsored dental insurance plan. Employee is covering his dependent on his dental plan. Employee's spouse/domestic partner, who is employed by private industry (not a State employee) is currently enrolled in her own private industry sponsored medical and dental insurance plans and is covering the employee and their dependent. Desired flexelect elections Employee wants to enroll in FlexElect for the Cash Option in lieu of his own State-sponsored medical insurance plan and keep his dental insurance as it is. Employee also wants to put $ per month ($, per year) into the Medical Reimbursement Account and $ per month ($, per year) into the Dependent Care Reimbursement Account. Employee must complete a STD. C to enroll in the Cash job function email database Option and a STD. R to enroll in the Medical and Dependent Care Reimbursement Accounts. Employee's Personnel Office must complete the "Agency Use Only" section of the STD. C and STD. R and ensure the rest of the form is completed accurately and submit to SCO. Current status Employee is currently enrolled in his own State-sponsored medical and dental insurance plans; employee covers his dependent on both plans. Spouse/Domestic Partner currently is enrolled in her own State-sponsored medical and dental insurance plans and does not cover any dependents. Desired flexelect elections Employee wants to enroll in FlexElect for the Cash Option in lieu of his own medical and dental insurance. Spouse/Domestic Partner wants to enroll in FlexElect and put $ per month ($, per year) into a Medical Reimbursement Account and $ per month ($, per year) into a Dependent Care Reimbursement Account. In addition, spouse/domestic partner wants to enroll employee and her dependent on her medical and dental insurance plans. Action required Employee must complete a STD. C and spouse/domestic partner must complete a STD. R to enroll in FlexElect. In addition the employee must also complete an HBD- and STD. to cancel his own medical and dental insurance; spouse/domestic partner must also complete an HBD- and STD. to add employee and their dependent to both her medical and dental insurance. Employee's Personnel Office must assemble the employee's STD.  C, HBD- and STD. ; complete the "Agency Use Only" section of each form; and ensure that all forms are completed accurately. Spouse's/Domestic Partner's Personnel Office must assemble the spouse's/domestic partner's STD. R, HBD-, and STD. ; complete the "Agency Use Only" section of each form; and ensure that all forms are completed accurately. One Personnel Office must coordinate submission of both the employee's and the spouse's/domestic partner's forms to SCO. The package should be assembled so that the STD. C and STD. R are on top.
