State-sponsored medical insurance plan but he is currently enrolled in his own State-sponsored dental insurance plan. Employee is covering his dependent on his dental plan.
Employee's spouse/domestic partner, who is employed by private industry (not a State employee) is currently enrolled in her own private industry sponsored medical and dental insurance plans and is covering the employee and their dependent.
Desired flexelect elections
Employee wants to enroll in FlexElect for the Cash Option in lieu of his own State-sponsored medical insurance plan and keep his dental insurance as it is. Employee also wants to put $ per month ($, per year) into the Medical Reimbursement Account and $ per month ($, per year) into the Dependent Care Reimbursement Account.
Employee must complete a STD. C to enroll in the Cash job function email database Option and a STD. R to enroll in the Medical and Dependent Care Reimbursement Accounts.
Employee's Personnel Office must complete the "Agency Use Only" section of the STD. C and STD. R and ensure the rest of the form is completed accurately and submit to SCO.
